We encourage (but do not require) our member agencies to submit records to our office for any in-service training they may hold at their department. This benefits your officers in that they can receive a certificate for the training they receive, and all of their training records are accessible in one location. It benefits the MTU by increasing our training figures for the year. Since many of the instructors within the MTU 10 region have received their instructor certification through courses offered by the MTU, this helps us to show a return on our investment for bringing in these costly instructor courses.

For these in-service courses, we are also required by our auditors, in addition to the forms below, to obtain a course outline/lesson plan, a copy of any handout material, and instructor biography for our files. Please include this in the packet of information for your in-service course.

(Please note: There has been some confusion regarding in-service training which is submitted to this office. MTU #10 cannot PAY for your department's in-service courses unless they have been requested via the annual training needs assessment survey and have been scheduled and budgeted by THIS office. Our budget is planned a year in advance and money is very tight. This leaves us little room for last-minute scheduled courses.)

Course Evaluation Form Blank evaluation form for use by MTU departments who wish to report any departmental in-service training to our office for certificate & credit. We should have an evaluation form for each person listed on the sign-in sheet. Course Liability Waiver Blank liability waiver form for use by MTU departments who wish to report any departmental in-service training to our office for certificate & credit. This form must be filled out and signed by each officer in attendance at any course where physical injury is a possibility. Course Registration Form Blank course registration form for use by MTU departments who wish to report any departmental in-service training to our office for certificate & credit. We should have a registration form for each person listed on the sign-in sheet. This form is not required for Haz-Mat Refresher courses.	REGISTRATION FORMS Course Registration Form Register your officers for our classes using this form. Applicable for all courses which do not have a "special" registration form (Mandatory Firearms, Executive Development, GRACIE, Part-Time Basic Training, etc.) YOU CAN ALSO NOW USE OUR ONLINE REGISTRATION FORM IF YOU'D PREFER!! Mandatory Firearms Training (MFT) Registration Paperwork All of the forms you will need to provide to us to register your officer for Mandatory Firearms Training in one easy download. Contains: Form E, Certificate of Liability Insurance and MFT Application Form. Certificate of Liability Insurance must be filled out by your Department's Liability Insurance Agent. The other two forms can be filled out by anyone, but must be signed by the department's Chief or Sheriff. Please remember that the ORIGINAL FORM E must ALWAYS go to the Training Board. We just get a COPY. Part Time Basic Training Academy Application Packet (All inclusive) Contains all MTU & ILETSB required forms to hire a new part-time officer, request training waiver if necessary, enroll officer in PT Academy, prepare for POWER test, and request reimbursement of tuition upon completion of training. Instructor Training Course Registration Agreement It is the policy of the MTU #10 Advisory Board that all officers trained as an instructor in any subject, by a Train the Trainer course hosted by MTU #10, shall be required to sign an agreement stating that they will conduct a minimum of two courses during the two years immediately following the trainer course, at no cost to the MTU. Completion and submission of MTU #10 course paperwork is required for these subsequent courses. This paperwork can be downloaded from this page.

Mandatory Firearms Roster Form This form is to be used by the range master to record course and student information when conducting a 40 Hour Mandatory Firearms Training course. Chiefs' 20 Hours of Mandated Annual Training As of January 1, 2006, all Police Chiefs in Illinois are required to attend 20 hours of training on an annual basis.
